Background Little is known about the attitudes and practices of intensivists working in Lebanon regarding withholding and withdrawing life-sustaining treatments. The objectives of the study were to assess the points of view and practices of intensivists in Lebanon along with the opinions of medical, legal and religious leaders regarding withholding withdrawal of life-sustaining treatments in Lebanese intensive care units. Methods A web-based survey was conducted among intensivists working in Lebanese adult ICUs. Interviews were also done with Lebanese medical, legal and (...) religious leaders. Results Of the 229 survey recipients, 83 intensivists completed it, i.e. a response rate of. Most respondents were between 30 and 49 years old, Catholic Christians, anesthesiologists, working in Beirut. Ninety-two percent of them were familiar with the withholding and withdrawal concepts and 80% applied them. Poor prognosis of the acute and chronic disease and futile therapy were the main reasons to consider withholding and withdrawal of treatments. Ninety-five percent of intensivists agreed with the “Principle of Double Effect”. The main withheld therapies were vasopressors, respiratory assistance and CPR. Most of the respondents reported the decision was often to always multidisciplinary, involving the family, and the patient, or his advance directives or his surrogate and the nurses. The interviewees agreed there was a law governing withholding and withdrawal decisions/practices in Lebanon. Christians and Muslim Sunni leaders declared accepting those practices. Conclusion Withholding and withdrawal of LSTs in the ICU are known concepts among intensivists working in Lebanon and are being practiced. Our results could be used to inform and optimize therapeutic limitation in ICUs in the country. (shrink)

To achieve accreditation standards and train residents for clinical practice, ACGME placed a lot of emphasis on ethical competence and professionalism. A crucial requirement for enhancing the standard of future medical practice is ethics education. This study sought to identify the requirement for ethics knowledge in clinical training from the perspective of the residents and determine the most effective methods for education. A cross-sectional descriptive study was conducted between March and May 2023. Participants included Lebanese postgraduate medical students, 210 completed (...) a Google Forms survey via WhatsApp. A validated questionnaire adapted from a previous study was used, focusing on demographics, ethical experience, attitudes, educational goals, and training effectiveness. Analyses were performed on IBM SPSS Statistics software (version 26.0) and R software. 210 residents representing a range of specialties participated in the survey. predominantly in internal medicine (54.8%). As Medicine shifts from a paternalistic practice to a patient-centered approach, the concept of medical informed consent (IC) has evolved to safeguard patient autonomy. However, its current implementation still presents many challenges in clinical practice. We assessed the knowledge and attitudes of the general Lebanese population regarding the IC process as well as their sociodemographic and medical correlates. An anonymous online survey was distributed to the Lebanese population using social media channels. A sample of 500 adults with an average age (...) of 36.2 ± 13.5 years, including 319 females and 181 males, was recruited. Most of the respondents had a university degree (85.8%), reported previous hospital admissions (75.9%) and had signed an IC for surgical procedures (40.7%). Few participants were knowledgeable about IC Lebanese law. Variability in knowledge level was significantly related to gender and a previous hospitalization history. Positive attitudes toward patient autonomy (53.1%) and shared decision-making (57.5%) correlated with older age, female gender, graduate education, and a previous history of signing an IC document. Males were more likely to believe that IC has positive effects on health than females. This is the first study that provides novel findings regarding Lebanese peoples’ awareness of the ethico-legal components of medical IC. (shrink)

Tax compliance has been extensively researched. Yet, the classic question ‘why do people pay taxes?’ remains unanswered. In Jordan, tax evasion is widespread. The state and citizens have been trapped in a continuous hide-and-seek game, which has taken the form of a virtuous cycle. This paper investigates tax evasion along with the most noticeable features of the Jordanian tax system. It also highlights how the virtuous cycle of tax evasion has been established and what could possibly be a way out (...) of it. We argue that sociocultural values are the most prominent cause for tax evasion. That is, the procedures of state formation and functionality in Jordan have created mutual distrust between the state and its citizens. Having been raised in a community characterized by deep distrust, tax evasion is no longer considered immoral by substantial numbers of the Jordanian population. We argue that successive governments need to create a high-trust-based culture and formulate strategies that serve to acquire normative ethical values in order to enhance voluntary compliance. The way to accomplish this necessity is through enforcing the state’s transparency and accountability. The taxation system needs to concentrate on income tax rather than raise tax rates on essential commodities to collect substantial tax revenues. Thus, further reform of the taxation system along with a reduction in tax incentives need to be considered. Moreover, lowering the exceptionally high personal income tax threshold would bring Jordan in line with its peers, expand the base, and introduce more progressivity. There would also be a scope to introduce a minimum tax based on sales as a tool to address tax evasion. (shrink)

With the coronavirus outbreak, new and strengthened norms of plastic dependency emerged in the Middle East and North Africa region through the desperate demand for products like face masks and other personal protective equipment, highlighting the tradeoffs between health and the environment. While the rise in demand has been considered as temporary, behavioral barriers and misperceptions might make these norms particularly sticky and hinder society’s ability to transition to a circular economy. As some behaviors are often strenuous to change and require enforcement through traditional policy solutions, behavioral science offers complementary tools that will make policies more effective. Objective To determine the attitudes of Egyptian patients regarding their participation in research and with the collection, storage and future use of blood samples for research purposes. Design Cross-sectional survey. Study population Adult Egyptian patients (n=600) at rural and urban hospitals and clinics. Results Less than half of the study population (44.3%) felt that informed consent forms should provide research participants the option to have their blood samples stored for future research. Of these participants, 39.9% thought that consent forms should (...) include the option that future research be restricted to the illness being studied. A slight majority (66.2%) would donate their samples for future genetic research. Respondents were more favourable towards having their blood samples exported to other Arab countries (62.0%) compared with countries in Europe (41.8%, p<0.001) and to the USA (37.2%, p<0.001). Conclusions This study shows that many individuals do not favour the donation of a blood sample for future research. Of those who do approve of such future research, many favour a consent model that includes an option restricting the future research to the illness being studied. Also, many Egyptians were hesitant to have their blood samples donated for genetic research or exported out of the Arab region to the USA and European countries. Further qualitative research should be performed to determine the underlying reasons for many of our results. (shrink)

Through a close reading of the works of the Ancient Greek Cynics , the Marquis de Sade , and Friedrich Nietzsche , this dissertation explores "ethics of destruction" that undermine set goals and determinate approaches to the world and that confront dominant social-historical institutions while privileging an approach to philosophy as a way of living and of relating to the world. Ethics of destruction affirm difference and irreducible singularities and undermine inherited beliefs and traditions; they reject prescribed social values set (...) on universality, transcendence, and fixity, and embrace singularity, indeterminacy, and transformation. ;The ethics of destruction described in this dissertation involve practices that oppose any domination and that attempt to do justice to what is "hidden and forbidden" and "ex-communicated" in the name of what has been constructed as life. The Cynics, Sade, and Nietzsche engaged in different ethics of destruction that led them towards transforming themselves and towards "becoming tyche," "becoming chaos," or "becoming fatum" respectively. These ethics privilege the multiple, unlimited, and indeterminate at the basis of something that can no longer be called "ontology" but that can be designated as "ways of living." They also accentuate the permeation and stratification of an unlimited, or rather an ungraspable, multiplicity of sensations, feelings, emotions, events, interpretations, etc., at the basis of what can no longer be called "epistemology" but that can be designated as "ways of relating to the world." The particular ways of living and of relating to the world associated with ethics of destruction are shown to privilege practices that open pathways towards "multiplicity.". (shrink)

The paper will draw on Foucault’s the last College de France lectures of to present his exploration of Cynic self-transformative practices and self-subjectivizing “ways of living” associated with social and political transfor-mation of ontology - of “life” and not just the “world” - as politics of difference, otherness, and alterity. For Foucault subjectivity is a historical production shaped through discursive practices immersed with social practices, where the transcription of power relations reflects various forms of governmentality as well as different “regimes” (...) and “dispositifs”, combining various techniques, mechanisms, relations, effects etc.. Subjectivity is not a socially fixed determinate product but “techniques of living” that provide the foundation for the social and discursive practices and subjectivization and self-subjectivization processes, but as a foundational horizon that is open, contingent, and always shifting. Foucault’s late focus on the “care of the self” and “parrhesia” not only complements his analyses of regimes of truth or of mechanisms, techniques, processes, and powers associated with various forms of governmentality, but provides the final chapter of his social and political ontology. This study attempts to investigate the role of movie and TV series clips in enhancing EFL learners’ pragmatic competence by utilizing an experimental design. The sample of the study was 42 students from the English language department at Cihan University-Duhok, Iraq. The experiment lasted one academic semester. The participants’ English language proficiency, as determined by an IELTS test sample, was intermediate, and then they were randomly split into two groups, namely experimental and control. Before and after the treatment, a Written (...) Discourse Completion Test served as a pre-and post-test given to the two groups to assess statistically significant differences between them. The experimental group received direct instruction on request and suggestion speech acts via the presentation of the carefully chosen movie and TV series clips. In contrast, the control group was exposed to a minimal amount of pragmatics through printed texts. The findings demonstrated that the experimental group outperformed the control group. More precisely, the findings revealed that movie and TV series clips had a significant influence on learners’ production of requests and suggestions. Considering the above findings, the researchers propose EFL teachers apply movie and TV series clips to improve their Students’ pragmatic competence in class. (shrink)

This article explores Amartya Sen’s understanding of freedom, and performs two central functions, one classificatory and the other substantive in nature. First, I situate his reflections within canonical understandings of liberty, finding an irreducible pluralism incorporating positive liberty in ‘capability’ alongside negative and republican liberty in ‘process’, which is subsequently unified in the notion of ‘comprehensive outcomes’. Secondly, I attempt to find a normative referent for the intrinsic value of choice, and thereby indirectly that of freedom, in his account. In (...) contrast to the liberal subjectivity one might – I believe, mistakenly – attribute to Sen’s deployment of neoclassical economic frameworks, I instead argue for a re-interpretation of his account, inspired by the sociological literature on embodiment. Ethics issues in the areas of science, technology and medicine have emerged during the last few decades. Many countries have responded by establishing ethics committees at the national level. Identification of National Ethics Committees (NECs) in the Eastern Mediterranean (EM) region and the extent of their functions and capacity would be helpful in developing capacity building programs that address the needs of these committees. Accordingly, we conducted a survey to determine the characteristics of existing NECs in the EM region.

ABSTRACT Ethical leaders can influence followers’ ethical behaviors by establishing an ethical climate. However, followers’ responses to an ethical climate may also differ according to the amount of attention they devote to moral questions. This study analyzes whether moral attentiveness augments the positive effect of an ethical climate on employees’ ethical behaviors, as well as the indirect effect of ethical leadership on employee ethical behavior through an ethical climate. Data from 270 employees in the Malaysian manufacturing industry indicate that the (...) positive impact of an ethical climate on ethical behavior is greater among employees who exhibit high rather than low moral attentiveness; this moderating role also applies to the relationship between ethical leadership and employee ethical behavior through the ethical climate. This study thus sheds new light on the notable role of moral attentiveness in ensuring that ethical leadership and ethical climate enhance ethical behavior in the workplace. (shrink)

Guilt and fear today have developed an unexpected quality: they contribute powerfully to the survival of humanity. The feeling of guilt proceeds from an elementary awareness: although the unequaled progress of science and technology in the twentieth century has undoubtedly ameliorated the conditions of human life, it also has given rise to an infernal logic of genocide and crimes against humanity, in which almost all nations, directly or indirectly, have participated and participate still. This awareness is joined to another, which (...) is itself accompanied by a primordial fear: for the first time in history, science and technology have endowed humanity with the power to destroy itself and the planet, without furnishing humans with the means of escaping their destined role as sorcerer's apprentice, for no science can tell us what to do with science. It is this double feeling of guilt and fear that pushes contemporary man to search feverishly for ethical foundations capable of furnishing discreet regulatory principles to underpin decisions and actions. From the point of view of contemporary humanity, ethics must be able to confer meaning and perspective on an existence which apparently has neither: “Now,” writes Hans Jonas, “we shiver in the nakedness of nihilism in which near-omnipotence is paired with near-emptiness, greatest capacity with knowing least for what ends to use it.”. Ten years ago, an issue of the journal L'Homme et la Société was entitled “La Mode des identités.” Most of the articles were about cultural identity, but the basic thrust of the work was “a critique of the ‘fashion’ of identity calling into question the validity of a notion as striking as it is uncertain.” Recently, a special issue of the journal Sciences Humaines, entitled “Identité et identités,” dealt with personal, familial, social, professional identities and only spoke of cultural identity (...) marginally and in a roundabout way. Finally, in a manual of Sociologie politique published by the Presses Universitaires de France, the author takes to task the notion of ethnicity inherent to cultural identity and pushes the critique of the ethnic group to the point of negating its objective existence. Must we think then that the notion of cultural identity is henceforth obsolete, that it has lost all pertinence? (shrink)

Background:Healthcare organizations are now challenged to retain nurses’ generation and understand why they are leaving their nursing career prematurely. Acquiring knowledge about the effect of ethical work climate and level of perceived organizational support can help organizational leaders to deal effectively with dysfunctional behaviors and make a difference in enhancing nurses’ dedication, commitment, satisfaction, and loyalty to their organization.Purpose:This study aims to determine the relationship between ethical work climate, and perceived organizational support and nurses’ organizational commitment, job satisfaction, and turnover (...) intention.Methods:A descriptive correlational research design was conducted in all inpatient care units at three major hospitals affiliated to different health sectors at Alexandria governorate. All nurses working in these previous hospitals were included in the study (N = 500). Ethical Climate Questionnaire, Survey of Perceived Organizational Support, Organizational Commitment Questionnaire, Index of Job Satisfaction, and Intention to Turnover scale were used to measure study variables.Ethical considerations:Approval was obtained from Ethics Committee at Faculty of Nursing, Alexandria University. Privacy and confidentiality of data were maintained and assured by obtaining subjects’ informed consent to participate in the research before data collection.Findings:The result revealed positive significant correlations between nurses’ perception of overall ethical work climate and each of perceived organizational support, commitment, as well as their job satisfaction. However, negative significant correlations were found between nurses’ turnover intention and each of these variables. Also, approximately 33% of the explained variance of turnover intention is accounted by ethical work climate, organizational support, organizational commitment, and job satisfaction, and these variables independently contributed significantly in the prediction of turnover intention.Recommendation:Strategies to foster and enhance ethical and supportive work climates as well as job-related benefits are considered significant factors in increasing nurses’ commitment and satisfaction and decreasing their turnover intention. This paper proposes a conceptual framework for understanding the implementation process of a complex intervention concerned with professional role change. The proposed framework holds that the intervention must address three interacting systems (socio-cultural, educational and disciplinary) through which a health professional role is evolved. Each system is operationalized by four dimensions (values, methods, actors and targets). As for the implementation, the framework posits that it can be analyzed, by depicting the barriers and facilitators located within the dimensions of the three (...) interacting systems and within the intervention involved in the process through using the “menu of constructs” approach suggested by the Consolidated Framework for Implementation Research (CFIR). Parochial altruism is manifested in the most violent of conflicts. Although it makes evolutionary sense for kin, many non-kin groups also behave parochially altruistically in response to threat from out-groups. It is possible that such non-kin groups share a sense of “fictive” kinship which encourages them to behave parochially altruistically for each other’s benefit. Our findings show that individuals not directly involved in a conflict approved of parochial altruism enacted by an in-group against an out-group more when the out-group posed (...) a threat to the in-group; however, this effect wasgreaterwhen the in-group members expressed fictive kinship by addressing each other using kinship metaphors such as “brothers.” Furthermore, although males approved of parochial altruism more than females, as the male warrior hypothesis would suggest, the effects of threat and kinship metaphor on approval of parochial altruism applied to both genders. These findings were replicated in an honour and non-honour culture. (shrink)

The “Averroism” of Ibn Ṭumlūs (d. 620/1223) has already been attested to by both biographers and textual evidence. Conversely, a close reading of a section in his Kitāb al-Qiyās of his Mukhtaṣar fī al-mantiq (Compendium on Logic) reveals that this “Averroism” has a specific resonance. Unlike Ibn Rushd (d. 595/1198), Ibn Ṭumlūs belongs to this group of philosophers who dedicated a special section to the examination of jurisprudential inferences, titled by him “al-Qawl fī al-maqāyīs al-fiqhiyyah”, which is a part of (...) his Kitāb al-Qiyās. In this article, first, I will introduce the text of Ibn Ṭumlūs, and present its teaching by placing it in its intellectual context, namely in the light of al-Fārābī’s, al-Ghazālī’s, and Ibn Rushd’s relevant texts. Then, I provide an English translation followed by a critical edition of the Arabic text. The purpose is to highlight the value (and limitations) of Ibn Ṭumlūs’s effort, based on al-Fārābī (d. 339/950), al-Ghazālī (d. 505/1111) and Ibn Rushd, to apply Aristotelian syllogistic model to the jurisprudential inferences.
